If you wish to appoint more than one Authorised Representative, please complete one Authorised Representative Form for each person you wish to appoint.<\/span><\/p>\n<p style=\"padding-left: 40px;\">For security reasons we require you to submit the completed Authorised Representative Form to us as a signed original and witnessed by one of the following persons below:<\/p>\n<ul>\n<li style=\"list-style-type: none;\"><ul>\n<li>A Justice of the Peace;<\/li>\n<li>An Accountant who is a member of the Australian Institute of Chartered Accountants, CPA Australia or the National Institute of Accountants with 2 or more years of continuous membership;<\/li>\n<li>A Solicitor or Barrister;<\/li>\n<li>A Police Officer;<\/li>\n<li>An agent in charge of, or a permanent employee (with 2 or more years of continuous service) of an Australia Post outlet.<\/li>\n<li>An officer with, or authorised representative of, a holder of an Australian Financial Services Licence, having 2 or more continuous years of service with one or more licensees.<\/li>\n<li>A Dentist<\/li>\n<li>A Pharmacist<\/li>\n<li>A Medical Practitioner&nbsp;<\/li>\n<li>A Chiropractor or a Physiotherapist<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<div>&nbsp;<\/div>\n<p style=\"padding-left: 40px;\">Please contact us on <a href=\"tel:1300001240\">1300 <\/a>553 678&nbsp;(Option 4) if this proves too difficult or inconvenient for you, and we will work with you to find an alternative way of appointing an Authorised Representative.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>Appointment of an Authorised Representative or Advocate If you wish to appoint an Authorised Representative or Advocate to deal with eephone on your behalf, please complete the form below.
